Stevia rebaudiana is native to Paraguay and parts of Brazil. Indigenous Guaraní communities used the leaves—called ‘ka’a he’ê’—to sweeten mate for centuries. Modern isolation of its sweet steviol glycosides began in the 20th century, with rebaudioside A (Reb A) becoming the first widely commercialized, high-purity stevia sweetener in the U.S. after FDA ‘no questions’ GRAS letters in 2008–2011. EFSA approved steviol glycosides for the EU in 2011 (E 960; now E 960a for leaf-derived), and JECFA set a global ADI expressed as steviol equivalents.

Potential Concerns
The main safety guardrail is the ADI of 0–4 mg/kg bw/day as steviol equivalents; while average intakes are well below this, small children who consume many stevia‑sweetened products could approach it. Not all products labeled “stevia” are the same—only purified steviol glycosides (such as Reb A) are permitted as sweeteners in the U.S.; whole leaves or crude extracts are not. Research on long‑term microbiome effects is ongoing, but current evidence and human metabolism data show no safety issues at permitted intakes. Taste can include a bitter or licorice‑like aftertaste at higher levels, leading manufacturers to blend sweeteners or use flavor modulators.

Reb A lets manufacturers substantially cut added sugar and calories while keeping sweetness, which supports weight and glucose management goals when used in place of sugar. It contributes no digestible carbohydrates at use levels, so it has minimal impact on blood glucose and insulin. Typical use levels are very low—on the order of 50–600 mg per liter in beverages and 100–1000 mg per kilogram in foods—often blended with other sweeteners for taste balance. It is heat‑ and pH‑stable enough for many beverages, yogurts, confections, and baked goods.
Digestive Effects
Most people tolerate Reb A well because intact glycosides are poorly absorbed; they reach the colon, are hydrolyzed to steviol by gut microbes, then conjugated to steviol‑glucuronide and excreted in urine. This means typical doses do not cause the osmotic laxation seen with some sugar alcohols. Occasional users may notice a lingering or slightly bitter aftertaste rather than gastrointestinal symptoms. As with any intense sweetener, very high single‑product use is more likely to cause taste issues than gut effects.
Limit Consumption
Infants: high‑intensity sweeteners are generally not allowed in infant formula and many infant foods; U.S. GRAS notices for steviol glycosides exclude infant formula, and Codex food category permissions vary by product type. Children: because of lower body weight, heavy consumers could approach the ADI; moderating frequency and portion sizes is prudent. Pregnancy and lactation: major evaluations (JECFA/EFSA) have not identified reproductive or developmental risks at permitted intakes. People with diabetes can use Reb A to replace sugar; focus on the overall diet quality and watch for added bulking sugars in some products.
