As a steam-distilled, refined essential oil, peppermint oil concentrates volatile compounds, including menthol and trace levels of pulegone and menthofuran. Those two are restricted in the EU due to liver-toxicity concerns, and quality suppliers rectify and test oils to keep them within legal limits. For most consumers, the amounts used in foods are very small and well below safety benchmarks. The most common user-level issue is reflux or heartburn in sensitive people, especially with strongly flavored mints or gums. Swallowing large amounts of the concentrated oil is unsafe, but that is outside normal food use.

Digestive Effects
At flavor use levels, most people notice no digestive effects. In people with GERD or a sensitive esophagus, menthol can relax the lower esophageal sphincter and trigger heartburn or reflux. Symptoms are more likely with products that have a higher mint load, such as strong mints or some gums, than with lightly flavored foods. Nausea and vomiting are described mainly after high, non-food doses of peppermint oil. If mint flavors trigger reflux for you, choose milder flavors or avoid them.
Limit Consumption
People with GERD or frequent heartburn may feel worse after mint flavors and may prefer to avoid strong mint products. Infants and young children should not be exposed to concentrated essential oils; trace flavor in foods is typically not an issue. Those concerned about liver health should know that the EU limits certain mint oil constituents (pulegone and menthofuran), and compliant products keep exposures low.
