Industrial liquid fructose emerged in the late 20th century with enzymatic starch conversion: corn starch is first turned into glucose syrup, then isomerized to fructose and fractionated to produce high-fructose streams. One such stream (≈90% fructose) is marketed as 'liquid fructose' and used directly or blended into other syrups for beverages and foods.

Fructose absorption varies widely. Test doses around 25 g of free fructose are commonly used to detect malabsorption and can cause gas, cramps, and diarrhea in sensitive people. Symptoms are more likely when fructose exceeds glucose in a food or drink; pairing fructose with glucose often improves tolerance. People with IBS or functional GI disorders are more likely to report symptoms at typical beverage‑sized doses.

People with hereditary fructose intolerance must strictly avoid fructose, sucrose, and sorbitol to prevent severe hypoglycemia and organ injury. Individuals with IBS or SIBO may experience GI symptoms from large single doses of free fructose; splitting servings or pairing with glucose may help.
